Region 9 Volunteer Registration
ALL AYSO volunteers (coaches, referees,
teams managers, etc) are required to register each year before
they are allowed to be with our boys and girls.
On-line registration
is available through the eAYSO system (the same system you may have already used
when PreRegistering you child or children on-line).
Or, paper forms
will be available at all Volunteer training classes and meetings.
Whether
on-line or paper, you must complete ALL information on the form, sign
the printed form, and present the form along with your Drivers License to
the Region 9 Child and Volunteer Protection Advocate (CVPA) or other designated member
of the Region 9 Executive Board.

Registration
Please read these instructions carefully!!
Note: Submitting an on-line volunteer
application (pre-registration) does NOT mean that
you are registered as a Volunteer in AYSO Region 9. You are registered only
after our Child and Volunteer Protection Advocate (CVPA) has checked your
information for completeness and signed off on your application.
To register as a Region 9 volunteer, you have a choice:
- You can register the old-fashioned way (get a paper
application from your team coach or at one of the Coach, Referee, or Team
Manager meetings; fill out and sign the form, wait in another line, etc.;
click here for more details); or
- You can fill out your Volunteer Application(s)
on-line! All you need is a computer with internet access: at home, at work,
at the library, at a friend's!

Whether you choose the on-line or paper
application, you must:
- Make sure that each Volunteer Application is completed
and signed by you, the volunteer.
- Provide proper identification (drivers license or other
state ID) to the CVPA.

On-Line Application
- You can skip to step 4 if you have previously used
eAYSO to register a child to play and/or to register yourself as a volunteer
in a prior year. Go to
http://www.eayso.org/ and select Click here to get a Username &
Password.
- The next screen is the Parent Registration form. Even
if you are not the parent of a Player, provide your name, contact information
and your e-mail address. Your Username will be your e-mail address; choose a password. Be sure to
select a password that you will be able to remember in a week or
in a year (so that you will not have to enter everything from scratch next
year!)
- Once you have completed the Parent Registration a
screen will appear which states: “Thanks for Registering." Click on the
Back button.
- You can now login to the system with the username and
password that you entered in step 2 above.
- You will then find a Welcome screen with your name.
Click on the My eAYSO link in the upper left portion of the screen and
the Pre Registration menu will appear.
- Click on Edit Member information and ensure that
all the information is correct. Click Save
- Click on View/Edit Parent Information. Complete
all fields (again, even if you are not the parent of a Region 9 player).
Those fields marked with red stars (ex, Relationship to child) are REQUIRED
fields.
- Click on the big blue AYSO Volunteers button,
then the Click Here button next to "Sign up as a NEW Volunteer".
Special note for Youth
Volunteers. Complete the
required fields for the Volunteer form. All red star marked fields are
required.
- This system requires Acrobat Reader 6.0 to print
forms. Click on this
Acrobat link to download and install Acrobat Reader if it is NOT on your
system.
To Print the pre registration form, click on Print Pre registration form.
Select Volunteer form and click on Print/Preview Forms. You will
see a print icon on the top left corner. If you have a printer attached to
your computer click on the printer icon and print two copies of the form. You could also
save the form in your computer to print it later.
- Please check the contents of the printed form. Use the
View/Edit option in My eAYSO to update Volunteer information and
then print the form again.
- Sign and date two(2) copies of your printed
Volunteer Application (print a 3rd for your own records, if you wish) and
bring both with you to one of the Coaches, Referees or Team Managers meetings
(see the
Region 9 calendar for dates, times and locations).
- For Region 9 Board Members: you must be an
eAYSO-registered Volunteer before the Regional Commissioner can grant you
access to other eAYSO functions necessary for your job.

Paper Application:
If you are unable to register on-line, you can still
sign-up the old-fashioned way: paper and pen.
Blank forms can be picked up at one of the Coaches,
Referees or Team Managers meetings (see the
Region 9
calendar for dates, times and locations) or from your Team Coach.
Please fill out each Volunteer Registration form
COMPLETELY and LEGIBLY! If we can't read the form, or if the information is
incomplete, we cannot register you as a volunteer!
The following fields are REQUIRED
on the Volunteer Registration Form:
Region # (you are volunteering for Region 9)
First and Last name
Complete Address (address, city, state, zip)
Home Phone
Your Gender
Your Social Security Number (displayed on-screen and when printed as stars for
security reasons!)
Your Birth Date
Your Driver's License or Other State ID # (when printed, only the last 4
characters will display)
Your Role in Region 9 (coach, referee, team parent, etc.)
Past AYSO experience
Complete Personal Reference information
Complete Professional OR Prior Volunteer Reference information
Yes or No answers to 2 Disclosure questions
A Full Description if you answered yes to either Disclosure question
