2010 Player Registration
Pre-Register Players and Volunteers at
www.eayso.org
Please read these
instructions carefully!!
Note: Submitting an on-line player Application does NOT mean that your child is registered in AYSO Region
9. Your child IS registered only after our Registrar has received your
payment and checked your information for completeness.
On-Line Application

Go to http://www.eayso.org/
and click on Log In if you have used eAYSO in the past (and remember
your Username (typically your e-mail address) and Password; try
Forgot Password if you can't remember).
Select First Time if you have not used
eAYSO before. Click here to make sure your
computer and your internet browser can be used with eAYSO.
- For First Time users the next several screens will give you some
basic information about using the eAYSO website.
- When you get to the
My eAYSO screen (screen 1), click on ...
the appropriate link under "Apply for my children to play"; or
the appropriate link under "Adults wanting to volunteer" or "Youths
wanting to Volunteer" for volunteers under age 18.
The process for signing up a player is as follows:
- Select a Region; Thousand Oaks is
Region 9
- Enter information about you and your child(ren). Any asked-for
information marked with a red star (*)
is required; you cannot continue with
your Application until all such fields have valid information.
- Print out and sign 2 copies of the Player Application(s); you may want an
additional copy for your records. BEFORE TRYING TO PRINT THE
APPLICATION FORM(S) CHECK TO MAKE SURE THAT POP-UP BLOCKERS ARE
OFF IN YOUR BROWSER!
- Sign and date all copies and bring
them to one of the two Walk-In Registrations April 17th or May 8th.
- Pay the registration fee of $125 per player, also at the Walk-In
Registration (cash, check or credit card).
- After you apply and pay, Region 9 must still accept the registration.
We try to accept every child, but we may refuse or cancel an Application,
e.g., if we have too many players, too few volunteers or not enough field
space.
Your child is NOT registered with Region 9 until he or she is
accepted by our Registrar; you will be notified by your child's
coach, a Division Administrator, or the Registrar.
Detailed instructions for using eAYSO can be found by clicking on Help in the
upper right portion of most eAYSO screens; you will need Adobe Reader to read
the My eAYSO User Guide.
Every team in AYSO Region 9 must have a Coach, one or more Assistant Coaches,
a Team Manager, and a Team Referee. You know nothing about the game of
soccer? You've never coached or refereed the "beautiful game"? Not
to worry: Region 9 will teach you everything you need to know, for free!
Be sure to indicate on your child's registration which job or jobs you would be
willing to do for the children of Region 9.
So, once you have signed your child or children up to play, please take an
extra 5 minutes to register yourself (and your spouse!) as an AYSO Volunteer.
Bring your Volunteer Forms with you to our Walk-In Registrations.
If you are having problems with eAYSO, make sure that your computer meets the
following specifications:

BROWSERS:
This website works ONLY on Windows 2000/XP and Mac OSX using the
following three browsers – Internet Explorer version 6.0 and above,
Netscape version 6.2 and above, Fire fox version 1.0 and above. This
website does NOT work with Safari, Opera and other browsers, nor
with Linux. If you are Mac user and would like to download the Fire
fox browser, go to http://www.mozilla.com/firefox/all.html. Click on
the download link under Mac OS for the English language version

POP-UP BLOCKERS:
Allow pop ups for this website. Internet Explorer users click on
Tools – Pop up blocker – Pop up blocker settings. On the area
“Address of website to allow”, enter www.eayso.org and click on Add.
Fire fox users add the site after clicking on Tools – Options –
Content - Allowed sites. Your Internet Service Provider (ISP) may
have its own pop up blocker as well. Please turn this one off too. |

Paper Application at the registration sign-up:
If you are unable to register on-line, you can still sign-up the
old-fashioned way: paper and pen.
Blank forms can be picked up at a Walk-In Registration (see the dates above).
Please fill out each Player Registration form COMPLETELY and LEGIBLY!
If we can't read the form, or if the information is incomplete, we cannot
register your child(ren).
The following fields are REQUIRED on the Player Registration Form:
Region # (9)
First and Last names
Complete Address (address, city, state, zip)
Home Phone
Player's Birth Date
Player's Gender
Handicaps ...? yes or no
Emergency Contact name and phone
Parent's First and Last names, Gender, complete address, relationship to child,
e-mail address if available
A mark by one or more Volunteer jobs: Coach, Ass't Coach, Referee, or
Other (i.e. team manager or other required team volunteers)
Parent or Guardian signature
